The Transition Network

TTN Chicago is proud to announce the launch of the Caring Collaborative to its members on March 28, 2022! This TTN Chicago member-only program supports the well-being of TTN members who are experiencing certain life challenges, medical concerns, or temporary needs due to illness, injury or disability. Several TTN Chapters across the country have added the Caring Collaborative to the services they provide members with tremendous success. We plan to do the same in Chicago.
Caring Collaborative services are designed to provide a safety net and to promote the healthy independence of our chapter members and consist of:
  • Service Corps—Members provide helpful services to other members such as home tasks, errands, companionship and accompaniment to health-related appointments.
  • Member Information Mix—Members who have had a particular medical condition or have been a care partner or have undergone a similar kind of personal challenge or transition share their experience on a confidential basis. The goal is to support the member’s decision-making process in three key areas: Medical Conditions, Provider Selection and Medical Products/Equipment that enhance comfort and mobility.
  • Caring Collaborative Presentations—Presentations on health-related and social/emotional wellness topics that will help members stay informed and independent. These will be presented by health care practitioners with expertise on the topic.

In order to participate in giving and receiving services, a chapter member must become a member of the Caring Collaborative program. Health and wellness programs and resources are currently open to members and non-members alike.

TTN Chicago will hold an information session via Zoom on January 27, 2022 from 6-7 pm. Members will have an opportunity to learn more about the services, get answers to questions they may have, and sign up for an orientation in early March. If you care about staying healthy and want the added resources offered by the Caring Collaborative that will keep you independent, you won’t want to miss this important information session. The Caring Collaborative is not intended to be a substitute for professional medical or counseling services, nor can it replace daily home care when that is necessary.
